This form allows you to select the organization ‘branch’ locations that will be able to view your application.  The screen should appear similar to below:

Your application will only be viewable by the sites that you select.  

To select a site, click on your selection in the list of available sites on the left hand side of the page and then click the '>>' button to move it to the list of selected sites on the right hand side of the page.  

To unselect a site, click on the site in the list on the right hand side of the page and then click the '<<' button. 

Our ‘demo’ applicant, Bill Smith, has authorized three sites to view and process his application.

To save your selections, click the ‘Save’ button or click 'Cancel' to exit the screen without saving your information.  You will be returned to the 'Application Overview' screen.
