Background Check
| Click the ‘Background Check’ link on the 'Application Overview' screen to display this application form. |
| The
‘Background Check’ application form contains information that will be
used by your organization/employer to perform background checks.
Information in this section is reviewed by an organization official
in strictest confidence and is only used to obtain information regarding
criminal history.
Once entered this information is “locked” and is no longer
available to the user or site officials.
Your date of birth is used ONLY for verification purposes in the
applicant registration process to avoid duplication of applications and
for the background check process.
Information such as social security number and driver’s license
number are used ONLY for the background check process.
To
view the privacy statement of your organization, click on the ‘Privacy
Statement’ link at the top of the page. Lines
that are marked with an * are items in which entry is required. Please enter your information carefully and then click the ‘Save’ button. After you click on the ‘Save’ button your information will be validated to make sure that it is complete and you will then receive a screen like the following: |

Please
check over your information carefully. If information is not correct, please click 'Cancel' and you will
be returned to the 'Background Check Information - Edit' screen. You will be able to make the necessary changes to the form. When your information is correct, click the ‘Accept, Save and Lock’ button. You will receive one final confirmation message: |

Clicking ‘OK’ on this message will save and lock your information and return you to the 'Application Overview' screen. |
